The Home Based Primary Care RN (HBPC) is a professional nurse. As progression is made in the nursing profession, it is expected that leadership skills and competencies will be sustained with expertise and broad-based clinical knowledge to effectively implement health promotion, management of acute and chronic illness, and care of the geriatric patient in a home setting. The HBPC nurse will work closely with other members of the HBPC team, including Social Work, Nutrition Care, Physical Rehabilitation, Pharmacy, and Psychology, as well as other healthcare professional in planning and initiating care. The HBPC RN is obligated to ongoing, comprehensive evaluation and improvement of clinical practice and health care delivery systems to meet the needs of Veterans in home-based settings. The HB
