The LPN in Primary Care/Float uses a care model known as PACT or Patient Aligned Care Team accountable and responsible for the delivery of care to Veterans in the Bay Pines VA Healthcare Primary Care Service and its affiliated Community Based Outpatient Clinics. The LPN has the skills, knowledge and determination for self-direction and concentrated effort to carry out the most complex practical nursing care assignments with patients ranging in age from young adulthood to the most senior years. The Licensed Practical Nurse will influence and support the flow of patients throughout their respective clinics.
