Job SummaryThe primary role of the behavioral health specialist is to co-lead Enhances Lifestyles for Metabolic Syndrome (ELM) group sessions during the intensive phase (first 6 months of the program) and facilitate maintenance sessions for the remaining 18 months.Job DutiesResponsibilities:Co-leads intensive phase 1.5-hour ELM group sessions at a Geisinger facility.Conducts one-on-one sessions with participant at the end of the intensive phase.Facilitates monthly maintenance sessions.Attends Intervention Committee meetings twice a month with the Intervention Coordinating Unit and weekly with Geisinger team.Coordinates with the Health Coach and Dietitian to provide on-going participant feedback between sessions.
