We are seeking an enthusiastic, patient centered family medicine physician assistant or nurse practitioner to advance LVHN's commitment to the community as part of LVHN's collaboration with Valley Health Partners.
Practice Overview/Job Responsibilities:
- Valley Health Partners Community Health Center (VHP) is a nonprofit organization recognized as a Federally Qualified Health Center Look-Alike (FQHC-LA) by Health Resources & Services Administration (HRSA). A FQHC-LA is a community health center that provides comprehensive primary care and other services such as interpretation, transportation and health education for the community population. An FQHC-LA does not turn patients away based on their ability to pay and removes financial barriers for receiving care. FQHC-LAs provide whole-person care, creating opportunities for the health center to go beyond the traditional four walls of medicine to address barriers that often are the root cause of poor health.
- The selected APC will be a member of the clinical team caring for patients in the Family Health Center, a practice which is part of Valley Health Partners.
- The dedicated Family Health Center team includes 11 physicians, a nurse practitioner, a physician assistant, 9 family medicine residents, a diabetic educator, embedded sports medicine, weight management, addiction specialist, social worker, care coordinator, pharmacist, and behavioral health specialist.
- This is the ideal position for a nurse practitioner or physician assistant who wants to be part of a teaching practice and work beside family medicine residents and medical students. You can use your knowledge, skills and compassion to make a difference in a community of underserved and underinsured patients with like-minded colleagues and professionals.
- This practice is located in Allentown, Pennsylvania.
Job Qualifications:
- A network champion for the underserved and a passion for clinical work
- A Valid PA Medical License or ability to obtain one is required
- Graduate from an accredited educational program for Nurse Practitioners
- Graduate of an accredited educational program for Physician Assistants
- Bilingual skills preferred but not required
