The Home Based Primary Care (HBPC) Registered Nurse Case Manager (NCM) is a professional nurse with broad-based clinical knowledge enabling him/her to effectively implement health promotion, prevention, and management for adult or older adult Veterans requiring skilled nursing intervention in the home environment. Basic Requirements: English Language Proficiency.
