Overview Atlantic Health System is seeking a full-time Complex Care Coordinator Registered Nurse for the Care Coordination department at the Corporate office to work 8AM-4PM including occasional weekends.Responsibilities Essential Functions/Responsibilities:Job Function 01: This R.N. Care Coordinator float position will span transitions of care (TOC) at all hospital campus locations as well as providing coverage to the Ambulatory practices within the ACO, and supporting remote patient monitoring (RPM) as needed.
