Job SummaryPerforms client assessment and triage including coordinating referrals to in-house clinical and community services, conducting family education, providing all aspects of nursing care, assisting physicians and physician extenders. Ensures that the client receives the mix of services appropriate to her needs in a timely fashion. Provides primary clinical support to the provider, patient and their family to assist in the diagnosis and treatment of the patient's condition.
