Job SummaryPerforms client assessment and triage including coordinating referrals to in-house clinical and community services, conducting family education, providing all aspects of nursing care, assisting physicians and physician extenders. The overall goal of care coordination is to ensure that the client receives the mix of services appropriate to her needs in a timely fashion. Provides primary clinical support to the provider, patient and their family to assist in the diagnosis and treatment of the patient's condition. Follows up on diagnostic testing to ensure completion, accuracy, and filing of same in medical record to ensure continuity and quality of care.Job DutiesEnrolls client into Healthy Beginnings Program and oversees the overall care of each clie
