JOB SUMMARYThe Clinical Liaison Registered Nurse (RN) initiates and develops relationships with current and potential referral sources and evaluates patients for post-acute care services. The nurse liaison performs pre-admission assessments to resolve patient candidacy for admission to the inpatient rehabilitation center. The nurse liaison works to ensure compliance with Medicare guidelines and proper documentation both on pre-admission and throughout the patient's rehab stay.ESSENTIAL FUNCTIONS OF THE ROLECompletes preadmission assessments timely, thoroughly, and with sufficient detail to justify a rehab admission.
