OPPORTUNITY HIGHLIGHTSSt. Peter's Health Partners is seeking Board Certified Nurse Practitioners with interest and experience in Geriatrics, Hospice, Palliative Care, and/or home healthcare to join the Home Based Primary Care team located in the Capital District, New York.Our Home Based Primary Care Team manages the care of homebound patients residing in Albany, Rensselaer, and southern Saratoga counties. Our team includes three Nurse Practitioners working with a collaborating physician experienced in the care of chronically ill/geriatric patients. One NP serves as the Practice Manager in addition to managing a caseload of patients, and the other 2 NPs have a caseload of assigned primary care patients within a designated geographic area/county.
