OverviewSt. Luke's Health System in Meridian, ID is seeking a Home Based RN Care Coordinator to join our Home Based Services team. Hours: Monday - Friday from 8:30am-5:00pm; with rotating on-call (phone support, no in person visits) The HBS Care coordinator supports our Home Based Services Providers (NP) in triaging patient needs, coordinating services with community providers and coordinating with the home health team.
