OverviewSt. Luke's Health System's Home Care team is looking for a full time PEDIATRIC Home Health Care Coordinator/Case Manager - RN to join our Boise area team! This is a direct care position providing in home care that requires traveling within the Treasure Valley area. St. Luke's Home Health is an extension of the patient's medical provider and the St. Luke's Children's Hospital. The home health team includes RNs, PTs, OTs, SLPs, RDs, and MSW. The team works collaboratively to take care of medically complex pediatric patients of all ages. The RN works as the primary RN for the patient and case manager to coordinate all care in the home and with the team of medical specialists in the Health System.
