Central Ohio VA Health Care System is seeking an Registered Nurse Float for Primary Care. Duty location is the Chalmers P. Wylie VA ACC. The PC RN Care Manager is a member of Primary Care whose role is to function as a team member, supporting and coordinating the nursing care to a panel of patients. S/he works in partnership with the assigned Primary Care Provider and panel of patients for effective disease and chronic care management. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
