Overview Atlantic Health System is seeking a full-time Transitions of Care Coordinator/Registered Nurse for the Care Coordination department at the Corporate office to work 8AM-4PM including occasional weekends.Responsibilities Essential Functions/Responsibilities:Identifies high risk patients for Hospital Value Based Programs with COPD/PNA. Completes bedside teach and supports hospital discharge plan of care in collaboration with physician and hospital care management team. Completes Transition of Care for high-risk patients for 30 days upon discharge to ensure continuity of care and quality healthcare delivery across the care continuum.
