Summary The Clinical Liaison functions as the liaison between patient, family (and when indicated legal representatives), referring hospital, and Delray Medical Center-Pinecrest Rehabilitation unit prior to admission. The Clinical Liaison is responsible for facilitating the delivery of the most appropriate post-acute rehabilitative services by accurately collecting data for pre-admission review, effective management of available resources, identifying an interdisciplinary treatment plan with realistic discharge goals of length of stay and outcome. Acts as a liaison between the referral source(s), physicians, Rehab Director, admitting office, nursing, inpatient case management and Manager of Psychology.
