This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. The certification must be in the specialty to which the individual is being appointed or selected. Preferred Experience: BLS is Required Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level III - Master's degree in nursing (MSN) and no additional nursing practice/experience. Nurse II - Master's degree in nursing and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: The position involves considerable standing, walking, stooping, bending, reaching, lifting, and turning. It requires restraining confused patients, repositioning patients, assisting with transferring patients to Wheelchairs/stretchers/cars, and transporting them to other departments or facilities. Mechanical devices available and are required to be used for heavy or difficult to lift patients. The work may involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. This position requires visual acuity, good hearing, distinctive speech and manual dexterity. The position requires the nurse to work for periods of time with arms above shoulder level with neck in a fully flexed or extended position and the ability to physically control or protect his/herself from physically abusive patients. The Psychiatric Mental Health Nurse Practitioner (APRN) is a member of the Access/Crisis Team (ACT) an interdisciplinary team in Outpatient Mental Health (OPMH) at the Salt Lake City Veterans Affairs Medical Center (VAMC). The ACT APRN is responsible for initial assessment, treatment, evaluation and disposition of veterans in crisis diagnosed with an acute and/or chronic mental illness. The ACT APRN is responsible for responding to crisis in the Emergency Department (ED), walk-in veterans in OPMH in acute crisis and requesting immediate treatment, telephone crisis/triage and other clinics/offices of the VAMC, as needed. Major Duties: Provide open access evaluations to veterans who are new to outpatient mental health clinic. Provide consultation evaluation for ED patients in mental health crisis. Facilitate admission, transfer and/or discharge from ED with appropriate continuity of care Provide education and/or teaching to veterans, families, staff and community agencies, as indicated. Provide education/training/mentoring to students, new employees, peer support staff, volunteers, as indicated. Consultation to members of the OPMH staff and professional staff throughout the VA and community, as indicated. Obtains and documents relevant psychiatric and medical history. Formulates the appropriate differential diagnosis based on the history, psychiatric examination and clinical findings. Orders medications, labs, consults, schedule appointments and utilizes evidence based screening tools to complete assessment, evaluation, interventions, and treatment strategies for veteran in acute crisis Prescribes medications which may include controlled substances per licensure. Establishes a treatment plan to achieve the identified health goals. As part of the evaluation, assesses veteran for suicidal/homicidal ideations and completes appropriate risk assessment and safety plan, as indicated. Collaborates with health care providers in assessment, triage, consultation, admissions, treatment and medication prescribing, as indicated. Utilization of motivational interviewing, screening tools, orders labs and consults as identified in initial assessment. Responds to ED for crisis consultation, inpatient admission coordination, outpatient treatment and disposition planning. Responds to requests from anywhere on VA campus to assess individuals for suicidal or homicidal ideation. Facilitates admissions both voluntary and involuntary to the Inpatient Psychiatric Unit (IPU) and/or collaborates for admission to community psychiatric resources. Makes appropriate referrals, as indicated, to reporting agencies such as Department of Family Services, Adult Protective Services, police, etc. Completes all required documentation according to VA and OPMH policies and Standard Operating Procedure (SOP) in a timely manner. Complete all required clinical reminders, annual TMS, mandatory skills, maintains current licensure and prescriptive privilege according to state, ANCC and VA guidelines. Participates in facility and/or community committees, staff meetings, discipline meetings, Quality Improvement, Research, as needed.
