The Care Coordination and Integrated Case Management (CC&ICM)-Registered Nurse utilizes an interdisciplinary approach to coordinate care required by patients with chronic, catastrophic, or complex high-risk or high-cost health issues. The incumbent has expertise in coordinated care and serves as the integration liaison in a partnership with the CC&ICM Social Work Co-Champion, between leadership and various programs and case managers/care coordinators. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
