This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The Registered Nurse (RN) General Mental Health (GMH) Case Manager (CM) is responsible for the coordination of care of patients receiving GMH services. He/she is responsible for providing patient-centered care within the nursing license scope of practice including initial assessment, intake and same day access appointments, completing clinical reminders, secure messages, patient callback requests, induction, stabilization, and maintenance phases of treatment of patients with a wide range of mental health disorders, including psychosis. He/she will collaborate with all mental health providers and other clinics (e.g., pharmacy, primary care provider, and specialty care) to whom the patient has been referred. He/she will coordinate between GMH providers and pharmacy to assist with prescription processing and refills. He/she will complete appropriate documentation in the medical record and comply with state, federal, and departmental policies when sharing/documenting patient care data. Through his/her well-developed clinical experience is able to anticipate and implement preventive patient care measures. The RN GMH CM will facilitate and coordinate the care of patients receiving GMH Services. The RN GMH CM will increase the use of provider time to focus on patient management. The RN GMH CM will improve access to GMH psychiatrists and nurse practitioners by providing daily management of complex psychosocial needs. Additionally, the RN GMH CM conducts reviews of the patient's medical record and other documents, ensuring that there is open communication between providers engaged in the care of Veterans. The RN GMH CM will work closely with a variety of healthcare professionals, clinical services, internal and external services, community agencies to provide efficient and effective health care using the appropriate resource management criteria. He/she acts on behalf of the Veteran to ensure that necessary clinical services are received, and that progress is made. In addition, this RN possesses the knowledge and skills to:\nA. Communicate and interact appropriately and courteously with all internal and external customers\nB. Maintain confidentiality of electronic, written, and/or verbal patient/employee information.\nC. Demonstrate working practices that include adherence to Infection Control standards and the safe use and operation of equipment.\nD. Maintains BLS certification and all mandatory reviews.\nE. Serves as the primary point of contact for GMH services.\nF. Works dynamically and cohesively with General Mental Health Section Chief, Mental Health Program Managers, and the Mental Health Department for implementation of programs to support General Mental Health.\nG. Functions as a navigator/case manager and liaison for planning, directing, and coordinating care for GMH Veterans.\nH. Monitor's clinic utilization and daily patient flow.\nI. Ensures adequate clinic support for providers.J. Reviews consults and facilitates scheduling in a timely manner.\nK. Triages telephone calls, secure messaging, and walk-in patients and offers same-day appointments.\nL. Collaborates with Telephone Care to schedule patients appropriately to maintain continuity of care.\nM. Maintains updated file of referrals and provides referral information to providers.\nN. Tracks high-risk patients and provides triage and education.\nO. Provides care coordination for high-risk patients and performs clinical assessments and interventions.\nP. Develops case or condition-specific plans of care utilizing clinical information system to establish short and long-term goals.
