The community you’ll be caring for: AdventHealth Lake Wales
The role you’ll contribute: The Social Worker III is responsible for the provision of psychosocial services for patients, their families, and patient experience within the Center for Behavioral Health. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
The value you’ll bring to the team:
Responsible for achieving and maintaining year-round Patient Experience scores in the 75th percentile or the targeted metric determined by AdventHealth
Responsible for reducing Length of Stay and avoidable delays in accordance with targeted metric established by AdventHealth Leadership; daily Length of Stay (LOS) tracking and report out to Social Services Supervisor
Completes Psychosocial History on each patient within 48 hours of admission, within 24 hours of admission for Medicare patients
Completes daily workflow and processes as assigned by the Social Services Supervisor or designee ensuring all tasks are executed in accordance with DNV, DCF, and AHCA standards, as well as, departmental protocols, AdventHealth Policies and Standard Operating Procedures (SOPs)
The expertise and experiences you’ll need to succeed:
Master’s Degree in Social Work or Counseling.
One year experience working with psychiatric or medical patients
CPI (Crisis Prevention Intervention) training is required within 30 days of employment
