The PACT RN Care Manager is a member of the PACT Teamlet that is composed of the Primary Care provider, RN, LPN, and MSA. RN manages a designated panel of patients and utilizes VA developed protocols to manage chronic diseases including hypertension, diabetes, COPD, CHF, and hyperlipidemia. RN performs nursing assessments which include bio/psycho/social status of the Veteran and assists in the development of comprehensive treatment plans incoordination with interdisciplinary team members. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
