The Post 9/11 Military to Veteran (M2VA) Program and Care Coordination & Integrated Case Management (CC&ICM) Manager is responsible for supervising and managing M2VA, Rural Access Network Growth Enhancement (RANGE) and Home Telehealth Remote Patient Monitoring (HT-RPM) programs. They are responsible for co-leading the systemic implementation of CC&ICM at the facility with the support of the Chief, Care Management and Social Work. ****This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30, 2022. The initial cut-off date for referral of eligible applications will be May 24, 2022.. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled.**** Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Must have a Valid Driver's License. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The position requires the ability to cover clinical nursing services and meet physical demands found in the patient care environment. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual Tuberculosis (TB) screening or testing, as a condition of employment. Initiates Basic Life Support (BLS) in accordance with current BLS standards to include automatic defibrillation in life-threatening situations. Work involves oversight of a complex array of services, both clinical and administrative, for all Veterans treated by the VA WCHCS. Work requires the exercise of mature professional judgment and flexible use of administrative, leadership, and nursing. The population served has unique needs, and the re-adjustment to civilian life, mental health issues, and the necessary involvement of family and other agencies often result in serious and complicated cases. Veterans present with a wide range of psychosocial and environmental problems that require creative problem solving and planning. Decisions concerning planning, organizing, and implementing the plans are often complicated by the various agencies and individuals involved in the care plan. Responsible for providing leadership to major programs across all campuses and providing direction to staff responsible for care coordination, care management and case management including establishing policy and procedure that ensures timely access to high quality care across the continuum. Works collaboratively with Executive Leadership , Service Line Managers, and other clinical and administrative staff in planning, developing, implementing and evaluating programs to support and enhance patient care across the continuum. Develops policy and procedure to promote timely and appropriate Veteran-centered care and interdisciplinary communication to ensure the right care, at the right time, at the right place, and at the right cost. Assists in developing and interpreting the mission, goals, policy, and performance improvement for care coordination, care management and case management services Facilitates efficient and appropriate delivery of services across episodes of care within the (VAMC) and (CBOCs), including Mental Health Service. Accountable for managing resources, supporting M2VA, RANGE, HT-RPM, CC&ICM and other Veteran-centered initiatives, research, and maintaining standards of practice to ensure high quality care across the continuum Plans, develops, implements and evaluates all programs under their purview to ensure continual quality improvement and excellence in customer service. The work affects a wide range of agency activities and operations. Manages, plans, implements, monitors, and evaluates the delivery of services utilizing CC&ICM. Coordinates interdisciplinary staffing, work assignments, budget, treatment services provided, and admission criteria for all three programs. Identifies areas for improvement, gathers relevant data, assesses the data, develops and implements ideas for improvement, and evaluates efficacy of improvements. Prepares the goals for each new fiscal year and reports on outcomes regularly and as requested by the Chief, Care Management and Social Work. Responsible for developing a systematic plan for reviewing and appraising the effectiveness of CC&ICM. Participates in program development and other special duties and projects as assigned by the Chief, Care Management and Social Work.
