As the Patient Care Coordinator you will demonstrate leadership in delivering and improving holistic care through collaborative strategies with others. This position requires flexibility in schedule and assignments, as this is a 24/7 hospital. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: -BLS -3-5 years Acute Care experience -3-5 years Care Coordination experience -CCCTM or CCM certification -BSN or MSN-prepared nurse -Patient Care Coordinator experience Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Physical Requirements: The position requires moderate to heavy lifting (44 pounds and over), pushing (1 hour), walking (4-6 hours), standing (4-6 hours), repeated bending (0.5 hours), both legs required, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, ability to distinguish basic colors, ability to distinguish shades of colors, hearing, mental and emotional stability, strong legs, and no skin allergies. ["VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Patient Care Coordinator is responsible for coordinating care in the inpatient setting and most importantly at transition from inpatient to outpatient, ensuring Veteran has resources in place to meet care needs. Under the direction of the appropriate supervisor, you will coordinate the interdisciplinary treatment and discharge planning of Veterans admitted to an acute medical/surgical/psychiatric unit, in conjunction with the interdisciplinary team. You will function as a coordinator for the Veterans' care, monitor Veterans' progress through the continuum of inpatient care, and coordinate discharge planning to transition the Veteran to the next level of care (outpatient, residential, community, etc.). As a consultant you will serve as a subject matter expert (SME) on clinical, educational, administrative issues and provide leadership across a range of contexts that contribute to organizational outcomes. You will work with staff throughout the healthcare system to develop corrective action plans which address deficiencies in practice and opportunities for improvement. Consultants are involved in education, development of practice, leading practice change and research. Use clinical knowledge to evaluate the quality and effectiveness of clinical performance within the facility, using data collected by internal/external monitoring processes and relay this information to clinical leadership and senior leaders. Facilitate recommendations for systems and/or resource changes that will serve to improve the work force and resource utilization. Play a critical support role in developing and coordinating internal review systems to assure that both clinical and administrative activities follow agency accrediting and regulatory requirements. Possess a thorough working knowledge of the directives, policies, and performance standards of the Veterans Health Administration (VHA) and of the Department of Veterans. You must be poised and articulate in communicating with people of varied educational levels and varied ethnic backgrounds. You must demonstrate willingness to work cooperatively as a member of a team in all assignments. You will make caring for the Veteran his/her priority while in the work setting and demonstrate customer service principles in all aspects of work. You must be self-motivated in the pursuit of meeting professional work standards.
