Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. ["VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Bruce W Carter Department of Veterans Affairs Medical Center is looking for a Registered Nurse (Case Manager) to work in the Mental Health Intensive Case Management (MHICM) Team. The MHICM program serves Veterans having a chronic serious mental illness for whom it has been shown that the intensive case management model is effective. The focus is on reducing the Veteran's use of inpatient psychiatric services and fostering his or her recovery. Approximately 80% of services are provided in the Veteran's home or within the outpatient community setting (20% in office/clinic setting). A substantial portion of the workload directed by this position may be carried out at one or more locations (including VA Community Clinics) that are physically removed from the main hospital under variable conditions. The RNCM provides medication management, which may include the administration of injectable psychotropic and opiate antagonist medications. Individualized social, clinical, and care coordination services are provided to Veterans who reside within a 30-mile radius of the Bruce W. Carter VA Medical Center. The RNCM practices with a great deal of autonomy but also functions as a part of an interdisciplinary team with VA staff from multiple services as well as community support networks for referral to resources. Intensive case management is employed through weekly face-to-face patient encounters (both in the community and in the MHICM office), supportive counseling, wellness recovery guidance, group work, crisis management, and recovery treatment planning. The RNCM at times assists to engage Veterans to participate in various recreational outings organized by the MHICM recreational therapist. Other duties include but are not limited to: Develops and implements an individualized, strengths-based, recovery-oriented plan of care to meet the Veteran's stated goals. This includes self-care treatment measures and re-integration into the community to promote highest level of function for the Veteran. Obtain a health history of referred patients; data collection includes lifestyle patterns, pain issues, sleep/activity levels, support systems, cultural, social, and ethnic factors, patient treatment preferences and goals. Provide psychosocial support, assist with problem solving and development of coping skills, and support the Veteran's personal goals for recovery. Administer long-acting injectable medications, including antipsychotics and opiate antagonists. Collaboration with pharmacy for LAI requests and picking up medications from outpatient pharmacy. Administer screening tools as indicated. Gather data and statistical compilations in accordance with Joint Commission, CARF, NEPEC and VA Medical Center policy and procedures. Monitor ongoing treatments for adherence, medication side effects, and clinical outcomes, including worsening of symptoms, impairment, and therapeutic response, and provide and/or coordinate appropriate follow up. Assess patients and provide additional support between visits with licensed independent providers (LIPs). Refer to, consult with, and coordinate care with other VA health care providers. Serve as liaison for Veterans in community hospitals and treatment settings to facilitate continuity of care. Coordinate with other agencies and make referrals to provide Veterans with necessary community services. Function as an advocate for Veteran self-determination. Provide Veterans and their caregiver(s) with education regarding their illness/diagnosis, medications, and treatment plan. Facilitate psycho-educational and support groups with Veterans and/or caregivers. Actively participate in team meetings and other MH meetings as appropriate. Document mental health care and treatment within prescribed guidelines in CPRS. Operate and maintain GSA vehicle according to national, VISN, and local policies. Provide coverage as needed for other staff on MHICM, and for other nursing staff throughout OPMH.
