Job SummaryResponsible for ensuring optimal quality, clinical and financial outcomes; networking, assuring availability and coordinating needed patient and family services, identifying and advocating for individual needs of patients, identifying and resolving patterns of variance having a negative quality, clinical and financial impact, and creating action plans and systems to optimize clinical and system outcomes.Job DutiesAssumes the role of clinical expert.Coordinates patient care placing rehabilitation and social service consults per protocol.Facilitates weekly multidisciplinary team meetings.Participates in direct patient care in a preceptor role.Acts as a preceptor or adjunct faculty for student nurses as needed.
