The RN will work in the Mental Health Division as a member of MH-PC Integration Team. This team is collocated in the Primary Care area. The RN works with the members of the MH-PC team to coordinate care of veterans referred from Primary Care for depression, PTSD, or other mental health diagnoses. The PCMHI RN will function as a Depression Care Nurse, providing care management services to those with a diagnosis of depression as well as monitor those with other Mental Health diagnoses. The PCMHI RN possesses the knowledge and skills necessary to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting. This broad based clinical knowledge enables the PCMHI to effectively implement health promotion and prevention interventions, manage acute and chronic states, assist veterans in the attainment of their optimal levels of health and functioning, and provide supportive measures as indicated for veterans and their significant others. The PCMHI RN possesses the knowledge and skills necessary to: Communicate and interact appropriately and courteously with internal and external customers. Maintain confidentiality of written, electronic, and verbal patient/employee information. Work and function within a safe manner including infection control and use of equipment. Assure that patient safety is paramount in all aspects of patient care. Integrate and apply pertinent aspects of the Central Ohio VA Healthcare System performance improvement program into his/her professional practice. Utilize nursing research and literature in practice. Demonstrate clinical expertise and knowledge of a recovery-oriented model of care to improve the quality of patient care, the use of resources and to facilitate compliance with internal and external requirements and standards. Possess critical thinking, problem solving and conflict resolution when collaborating with other members of the MH-PC Integration team and other Primary Care staff. Possesses the knowledge and skills to effectively apply all aspects of the nursing process and care management principles within a collaborative, interdisciplinary practice setting, which will then enable effective implementation of health promotion and prevention practices, management of serious mental illness and recovery planning.
