You will perform initial and on-going physical and nursing assessment according to accepted standards of nursing practice. Assess, plan, and evaluate patient care needs. Carries out physician orders. You will participate in treatment team conferences to assist in planning and revising goals, objectives and interventions appropriate to the age-related and problem-specific needs of each patient. Changes in patient's physiological, behavioral and medical status are recognized, reported and documented as they occur. Collaborate with the treatment team to revise goals, objectives and interventions appropriate to the changes in patient status. Monitor, record and communicate patient condition as appropriate utilizing computerized documentation systems. Instruct and educate patients and families. Assess and coordinate patient's discharge planning needs with members of the healthcare team. Implements nursing plan of care for assigned patients and conducts and/or co-leads group therapy sessions for patients. You will orient and mentor new staff members. Work Schedule: Full-time Primary tours 2330-8a, 0730-1600, 1530-2400.
