This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more Duties/responsibilities included but not limited to: - Provides effective education to residents and/or family members in relation to common disease processes, medication and/or prescribed treatment regime. - Effectively communicates appropriate care and pertinent events to team members that support continuation of care and enhances quality/safety for both residents and staff - Respects resident privacy and dignity in communication of information and sharing in care delivery handoff. - Partner with team members to resolve care needs that occur at shift change for resident comfort and satisfaction - Collaborates with appropriate staff regarding resident care needs provided and unresolved workload - Ensures environment is clean, safe and ready for incoming team - Actively participate in hourly rounding on assigned CLC unit to ensure that resident needs are meeting the area of personal, environmental, care planning and family contributions - Engage residents and encourage as well as, support patient and their families to participate in care and decision-making at the level they choose. The opportunity for individual to make personal choices related to their care is essential (Patient and Family Centered Care) - Accurately performs and supports evidence-based care practices that prevent skin break down/pressure ulcers and effective wound healing - Provide care and serve as a leader in the provisions and documentation of restorative care to residents across the CLC units - Utilizes established customer service satisfaction concepts (AIDET.Etc.) in communicating with residents about care, concerns and activities - Ensures proper medication administration resources are available to administer medications - Supports resident centered activities that engage families and significant others in meal preparation activities - Utilizes established documentation tools and rounding tools to accurately communicate care delivery within a timely manner of assuming resident care assignment and provision of care. - Participates in orientation, unit-based in-services and mandatory education activities in a self-directed manner collaborates with supervisors and colleagues to arrive at solutions that minimize organizational disruptions on any job-related issues as well as, in the care environment - Establishes patient care priorities that ensure safe and effective patient care - Demonstrates compliance with infection control practices such as, hand washing and wearing appropriate personal protective equipment Preferred Experience: 2 years of LPN experience preferred Work Schedule: 7:00pm - 7:30 am or 7:00am - 7:30pm - rotating including weekend and holidays (additional work schedules may become available) Telework: Not Available Virtual: This is not a virtual position.
