This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The RN Care Manager uses the nursing process and evidence-based practice to collaborate with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven care plan for life. The RN Care Manager is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. The RN Care Manager provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN Care Manager demonstrates leadership by serving as an advocate for patients, and a team leader with colleagues as she/he continues to enhance his/her own and the team's professional growth, development and practice. Major duties and responsibilities include, but are not limited to: Leads and organizes delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning Uses sound clinical judgment in assessing, planning, implementing, documenting, and evaluating nursing care, based on the ANA Standards of Nursing Practice Guides staff in the application of critical thinking and process improvement to improve patient care Anticipates and recognizes medical/surgical/psychiatric emergencies and initiates or leads others to implement appropriate and timely actions Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes Functions as an expert in clinical practice and/or areas related to the assigned roles and responsibilities Systematically evaluates current practice and formulates outcomes for groups of patients and/or organizational processes within area of expertise Work Schedule: Monday - Friday, 8:00 am - 4:30 pm (NOTE: Shifts may vary depending on patient care needs.)
