The Licensed Practical Nurse demonstrates technical proficiencies in initiating, performing and completing assigned duties in providing care to patient populations in the Mental Health Inpatient Psychiatric Unit of the Alexandria VA Health Care System. Duties performed include, but are not limited to the following: Assists with the admission of patients to clinical unit; interviews patient and/or family/significant other and completes section of admission data base; Identifies and meets physical, emotional, and social needs of patients by providing individualized nursing care and contributes to the patient's multidisciplinary treatment plan; Documents patient change of condition regarding response, physically and mentally, to medication and therapeutic interventions; Demonstrates knowledge, skill, and ability to recognize and respond appropriately to emergency situations and has the ability to defuse and de-escalate patients in psychiatric crises; Administers accurately medications per medical center policy, including narcotics. Recognizes the more subtle signs and symptoms of potential or actual drug reactions, promptly informs the RN of observed changes in the patient's condition and initiates prescribed interventions; Establishes, maintains, and documents therapeutic relationships with patients, such as reality orientation and one to one and /or small group interactions; Encourages and motivates patients to participate in the therapeutic community by fostering increased self-reliance and independence and by involving patients in their own treatment plan, including transfer and/or discharge planning; Performs specialized nursing care treatments as outlined in established nursing care procedures, such as suicide status, one -to-one, and /or dealing with combative patients. Environmental Factors: Working closely with others, working with hands in water, working with machinery with moving parts, protracted or irregular hours of work; Use stretchers to transfer patients, use patient lifts, if applicable.
