-Provides ongoing communication and collaboration with onsite Providers (Psychiatrists and Nurse Practitioners) to ensure well coordinated care within the program.
-Provides ongoing communication and collaboration with identified Primary Care Physicians and/or each patient's Medical Care Home personnel to ensure proper discharge planning and transitioning from Pathways to Wellness' care.
-Act as liaison between patients, their families and health care personnel to ensure efficient and prompt health care service.
-Completes required assessments on all new admissions.
-Administering patient injections.
-Checks vital signs, monitor blood pressure, pulse and body mass index each client visit.
-Devise care plans, which include client referral or admission, medication doses, monthly medications checks with physician, take vitals each visit to the clinic, administering therapies and treatments, evaluation of results and summation of the plan's effectiveness.
-Daily documentation and participates in the implementation of treatment plans for clients.
-Coordinate health care resources and other services for clients and may also provide mental health support services and medical advice.
-Responsible for the prompt and effective provision of health care services.
-Notes physician orders and follows through as needed.
-Assists physician with completion of medication check visits on returning patients as required.
-Assists with maintaining and monitoring accuracy of client medical records.
-Participates in Provider/Staff meetings, Treatment Team meetings and Continuous Quality Review meetings.
-Coordinates all laboratory procedures, including Infection Control responsibilities.
-Assists with coordinating clients' scheduled appointments/sessions with physicians.
-Contacts others involved in the care of patients regarding medication/medical treatment issues, as necessary. [i.e. case manager, family members, crisis staff, pediatricians, etc.]
-Conducts client medication education groups and provides individual client education when necessary.
-Other duties as assigned
-California license as a Registered Nurse.
-Master's degree in Psychiatric Nursing is preferred but not required.
-BSN Certification as a Psychiatric/Mental Health Nurse is preferred but not required
