Recruitment is open for a full-time Licensed Practical Nurse for the Homeless PACT (H-PACT) Oklahoma City, OK. The individuals selected for this position will be accountable to the Nurse Manager for their respective area. Functions as an independent Patient Aligned Care Team within Primary Care. H-PACT is a personal health care consultation with medical a provider in which treatment or intervention using advanced medical technology and/or procedures is delivered on an outpatient basis. It is a coordinated "medical home" specifically tailored to the needs of homeless Veterans. Veterans assigned to an H-PACT receive care from a dedicated team that includes a primary care provider, nurse, social worker, homeless program staff members, mental health provider and others. This team provides coordinated clinical care, including for underlying unmet care needs, case management, outreach services, housing and social services assistance during the high-risk transition periods while homeless, obtaining permanent housing, and for those Veterans at imminent risk of losing housing. Must meet regulatory qualification standards as identified in VA Handbook 5005, Part II, Chapter 3; be a citizen, have full, active, unrestricted licensure as a LPN in any state. Must be able to physically and mentally perform the functions of the position. 1) Evidence of strong positive interpersonal/customer service skills and ability to work effectively with multidisciplinary team members, patients, family members and significant others. 2) Demonstrated ability to communicate effectively, both verbally and inwriting. 3) Demonstrated ability to function as a self-directed, productive team member and adapt readily to varying patient care needs and fluctuation in workload with minimal RN supervision. 4) Demonstrated ability to observe, identify, document, and report deviations from the norm, includes responding appropriately emergent situations. 5) Basic computer skills required (experience with a computer and mouse); advanced skills preferred (Microsoft word, excel, etc). 6) Demonstrated integrity and dependability. 7) Ability to work flexible hours when required. 8) Min of three years nursing of successful practice preferred. Work Schedule: Routine hours M-F, 7:30 am to 4:30 pm with flexibility as needed to meet patient care needs. Financial Disclosure Report: Not required
