Licensed Practical Nurse Lakeland Medical Center Multiple locations throughout Lakeland, Winter Haven, and Auburndale, Florida Schedule: Mon. - Fri., 8am - 5pm
Maintaining a positive, helpful attitude, the professional we select will work with patients, families, providers and staff to promote timely access to needed care; provide daily continuity of care coordination; and coach patients, families, and caregivers to understand each patient’s care plan and self-care management responsibilities. This individual will use technology to schedule appointments and prepare medical records and provide guidance and instruction for patients as directed by medical providers.
Additional responsibilities include:
Providing general nursing care to patients in a clinical environment.
Administering prescribed medications and treatments in accordance with provider directives.
Using case management processes to ensure that quality care is delivered to the patient in the most efficient and effective manner across the healthcare continuum.
Engaging patients, families, and caregivers in understanding, setting, and monitoring patient-self-management care plans in a manner that is culturally and linguistically appropriate.
Conducting and documenting assessments of patient needs and resources for effective self-care management.
Providing timely communication with patients, making inquiries, executing follow-up actions, and helping to integrate information into the plan of care.
Developing and maintaining relationships among patients and their families, as well as the care team that supports access to their providers (medical homes).
Demonstrating flexibility, enthusiasm, and a willingness to cooperate while working with others in multi-disciplinary teams.
Maintaining the sanitation and cleanliness of exam rooms and stocking necessary supplies or materials.
Education, Experience, and Licensure
Successful applicants will each possess proof of graduation from an accredited LPN program, a current Florida LPN license, Basic Cardiac Life Support (BCLS) certification, and high school diploma or GED equivalent. Associate’s degree and IV certification preferred.
